For an individual to be assessed and treated, a referral must be made by a health care professional including but not limited to the following:

• Family Physician
• Occupational Therapist/Physio Therapist
• Social Work/Social Agencies
• Nursing
• Chiropractor

Other Referral Sources:

• Individual/Self Referral
• Insurance Companies
• Employer
